Canadian Patient Safety Week (CPSW) is an opportunity to hear about patient safety improvements across Manitoba and Canada and share resources to improve patient safety and the quality of care provided in Manitoba.

This year Shared Health and Manitoba Service Delivery Organizations (SDOs) are highlighting the theme of Safer Care for Older Adults.

Falling Through the Cracks: Greg’s Story

Falling Through the Cracks: Greg’s Story
 is a short film about Greg Price’s journey through the health-care system. The 30-minute film gives a glimpse of who Greg was and focuses on the events of his health-care journey that ended in his unexpected and tragic death. In spite of the sadness of Greg’s Story, the message of the film is intended to inspire positive change and improvement in the health-care system. The film will be followed by a Q&A with members of Greg’s family.

Intersectionality and Patient Safety: Making Connections to Improve Outcomes

Presented by Valerie Williams, Director of Equity, Inclusion, and Diversity, University of Manitoba’s Faculty of Health Sciences, and Hannah Moffatt, Provincial Lead for Learning and Sharing, Shared Health. This workshop introduces the concept of intersectionality and explores how can we leverage the concept to improve patient safety in Manitoba. Asking the Right Questions Leads to the Right Actions: The Importance of Frailty and Timelines When Providing Acute Medical Care to Older Adults

Presented by Dr. Rhynold (Prairie Mountain Health) &  Dr. St. John (WRHA) this Centre for Healthcare Innovation (CHI) Grand Rounds will focus on recognizing how the prevention and multifactorial treatment of delirium demonstrates the principles of tailoring care to meet the needs of older adults living with frailty. This session will also explain the Geriatric 5Ms and how the Clinical Frailty Scale© can be efficiently used to stage underlying frailty for older adults.

Caring for Older Adults with Cancer: An overview of Manitoba and Introduction of a New TOOLKIT for Oncology Nurses

The overall health of older adults through their cancer experience is heterogenous and is impacted by a multitude of factors, including functional ability, multimorbidity and co-morbidity, social context, and psychological condition. The Oncology & Aging Special Interest Group national working group has developed a TOOLKIT for General Oncology Nurses Caring for Older Adults with Cancer. Come learn about cancer in older adults and how you can use the toolkit in the care for older adults with cancer.
