The Geriatric Outreach teams of the WRHA include both the GPAT Geriatric Program Assessment Team (GPAT) and Geriatric Mental Health Team (GMHT) options. These teams continue to provide specialized geriatric assessments during the COVID-19 pandemic. Much of their work is being done through virtual assessments, and in home visits are completed when essential.
Anyone can make a referral! The outreach teams have an OPEN referral process. Anyone can call us, including health care providers, community groups, families and clients.
